　※以下の方は予約は不要です。
　　・東京キャンパスで学生健康診断を受診する社会人学生（東京キャンパス事前申込済者）
　　・理療科教員養成施設生
　　・胸部X線健診のみの職員（東京キャンパス事前申込済者）

電離放射線健康診断の検査省略について
Omission of Health Examination for Those Engaged in Ionizing Radiation Work

受診券の「本日の受診コース」に『電離放射線健康診断1』のみ記載され、以下１～６の全ての条件を満たす場合は、検査を省略し、問診票の提出のみでも構いません。
If only “電離放射線健康診断1” is listed in ‘本日の受診コース’ on your medical examination ticket and you are all of the following conditions(1-6) are satisfied, you can omit the inspection and only a medical questionnaire may be submitted.

【対象者　Eligible persons】
以下の全ての条件を満たす電離放射線従事者
Ionizing radiation workers who meet all the conditions below

  1. 新規登録・再登録者ではない（これまでも登録を継続している）。
  2. 前年一年間に受けた実効線量が5mSv未満。
  3. 今後一年間に5mSvを超える被ばくの恐れがない。
  4. 放射線業務による、眼と皮膚の自覚症状がない。
  5. 保健管理センターより「検査省略不可」の指示をされていない。
    （「検査省略不可」者には、各健診前にセンターから連絡致します）
  6. 学外施設利用者においては、学外施設の主任者から検査受診を要求されていない。
 受診券に 「電離放射線健康診断1」のみ記載されている方
  1. Not a newly registered radiation workers or those who have renewed their registration (has been continuously registered)
  2. The annual effective dose received in the previous academic year was less than 5mSv.
  3. No possibility of radiation exposure above 5mSv in the coming academic year.
  4. No eye or skin symptoms due to radiation work.
  5. You have not received instructions from the University Health Center that you cannot omit any tests.
    (If the inspection cannot be omitted, the Center will contact you before the Health Examination.)
  6. For users of facilities outside, you have not been required by managers of facilities outside the university to undergo the health examination.

【提出方法　How to Submit】

  1. 以下より『問診票』を各自プリントアウト（白黒可）し、ペンで記入。
    Print out the “Medical Questionnaire” from the link below and fill in the form with a pen.

  2. 登録施設の放射線管理室へ提出
    （附属病院は、病院総務部医療支援課診療施設事務係又は機能検査受付C201）
    ※受診券の提出は不要です。
    ※健診会場へは持参しないでください。
    Submit to the Radiation Management Department of their registered institution.
    (For the University of Tsukuba Hospital, Care Facility Section, Division of Medical Support, Department of University Hospital Management or the Functional Test Reception C201)
    ※It is not necessary to submit a medical examination ticket.
    ※Do not bring your form to the health examination venue.

特定業務従事者登録について
Registration of Specified Workers

今後１年間、勤務命令として、常時（週20 時間以上、深夜業務は月平均4 回以上を目安）下記特定業務に従事している方は、特定業務従事者として年２回健康診断を受けなければなりません（検査省略不可）。
Those who regularly (around 20 hours or more per week and for night work four times or more on average per month) engage in the following specified work as a work order for the next year must undergo medical examination twice a year as a person engaged in specified work (examination cannot be omitted).

【対象者　Eligible persons】
下記特定業務に常時（週20時間以上、深夜業務は月平均4回以上を目安）従事している職員
Employees who regularly (around 20 hours or more per week and for late-night work four times or more on the average per month) engaged in the following specified work

●以下については関係部署から指定されます。
　The following works will be designated as eligible by the relevant department.

Physicians and nurses in the Department of Infectious Diseases, Department of Clinical Engineering, Clinical Laboratory Department, Department of Pathology, THDC, Tsukuba Human-Tissue Biobank Center
深夜業務
Late-night work		 附属病院看護部、医師、附属視覚特別支援学校、附属聴覚特別支援学校
Nursing Department, Physicians, Special Needs Education School for the Visually Impaired, Special Needs Education School for the Deaf
騒音業務
Noisy work		 研究基盤総合センター技術室工作部門

低温業務

●上表以外で、特定業務に勤務命令として、常時従事している方は、以下の登録フォームより登録をお願いいたします。

大腸がん検診（便潜血検査）について
Colorectal cancer screening (fecal occult blood test)

【対象者　Eligible persons】
40歳以上の、常勤職員および週20時間以上勤務の非常勤職員のうち希望する者
Full-time employees and part-time employees who work 20 hours or more per week, who are 40 years of age or older, and wish to undergo the examination

【受付期間 Application period】
　～　2025年４月30日　　 – April 30, 2025

【受診方法　How to apply】
以下の申し込みフォームより登録してください。
健康診断開始の約１週間前に、受診券とともに容器が配付されます。
健康診断受診日に、受付にて受診券と２日分の検体を採取した容器を提出してください。
※ 受診日以外の提出はできません！ ご注意ください。

Please register using the application form below.
About one week prior to the start of the health examination, you will receive a container with the medical examination ticket.
On the day of the health examination, please submit the medical examination ticket and the containers with the samples for the two days to the reception.
※ Please note that you cannot submit the containers on days other than the day of the health examination! Please be careful.

Prostate cancer screening (PSA)

【対象者　Eligible persons】
50・55・60・65歳（令和8年4月1日時点の年齢）の、常勤男性職員および週20時間以上勤務の非常勤男性職員のうち希望する者
Full-time male employees and part-time male employees who work 20 hours or more per week, who are 50, 55, 60, 65 of age (Age as of April 1, 2026), and wish to undergo the examination.
