学生定期健康診断の心電図の再検査の予約ができます。
以下のカレンダーからご希望の日時を選択して予約してください。

【対象　Subject】
　保健管理センターよりメールなどで再検査の連絡を受けた学生
　Students who have received notification from the University Health Center
　regarding a follow-up examination by email or other means.

【実施期間　Implementation Period】
　令和８年５月　May, 2026　～　
　　　月（Mon）　13：15 ～ 14：30　／　火（Tue）・木（Thu）　9：00 ～ 11：00

　　※ 再検査２日前にメールでWeb問診をお送りします。来所前にご回答ください。
　　※ 予約ができない場合は、保健管理センターまでお問い合わせください。
　　※ 内科診察と並行して行いますのでお待ちいただくこともあります。
　　　 お時間に余裕をもってお越しください。
　　※ キャンセル・変更はHPではできません。保健管理センターまでご連絡ください。

　　※ The web medical questionnaire will be sent to you by email two days before your
　　　 re-examination. Please complete it before your visit.
　　※ If you are unable to make an appointment, please contact University Health Center.
　　※ The examination is carried out in parallel with the internal medicine examination,
　　　 so you may have to wait.
　　　 Please allow plenty of time for your visit.
